REFERRAL PROCESS

Please include the following in the email:
1. Face Sheet/Demographics
2. History & Physical (H&P)
3. Medication List
4. Wound information
- Location
- Description
- Photo
- Current Treatment Orders
5. Agency Notes (for the past 4 weeks)
6. Lab or X-ray Results (if any)

Please expect a response within the hour.

1. After receiving patient's demographics, we will verify eligibility.

2. Once eligibility is confirmed, we will send our one-time agency contract via DocuSign.

3. After we receive the signed contract, we will reach out to the patient within 24 hours.

4. Initial visit will be scheduled within 48 to 72 hours.

5. Notes will be provided 72 hours after the visit.
